Payment Alternatives:

Place your payment in one of the designated contribution envelopes and deposit it in the contribution box.  Alternatively, payments may be slid under Tyler's office door. 

** To ensure accurate processing, be sure to clearly label the envelope with the student's name and the name of the event. 

School Food/Beverage Parental Permission Form: 

If your teenager attends a Cabot public school and you would like Tyler to bring them a snack or drink during a lunch visit, please fill out the digital permission form by tapping the picture icon to the left.

Travel/Medical Consent Form:

Every family must complete the Travel/Medical Consent Form for their child(ren) who plan to participate in youth group activities. Please click on the image to the left to access the digital form.   

(Forms expire at the end of the year regardless of completion date, requiring a new form for the new year.)

Extracurricular Activities upload:

 With so many wonderful students in our youth group, I'm unable to attend every event, but I do my best to be present at a few throughout the year.  Please complete the attached form about your student's extracurricular activities so I can plan and attend when possible.  You may submit this form any time during the year, as some activities take place only in the fall or spring.
